ADHD Disability Concentrated at Ages 10–14 Across Asia, Study Finds

A comprehensive analysis of attention-deficit/hyperactivity disorder across 34 Asian countries has found that the disability associated with the condition is concentrated in childhood and early adolescence, peaking at ages 10 to 14. The study, published online in October 2026 in the World Journal of Pediatrics, matters because it moves the conversation beyond raw case counts and toward where, when and for whom ADHD imposes the heaviest functional cost.

Researchers from Kyung Hee University College of Medicine in Seoul and collaborating institutions used the Global Burden of Disease Study 2023 to examine ADHD-related years lived with disability from 1990 to 2023, comparing patterns by age, sex, region, socioeconomic development and pandemic period. The findings reveal marked heterogeneity rather than a uniform Asia-wide picture. East Asia carried the highest regional burden and followed an upward trend, with Japan and China recording the largest national estimates. Southeast Asia moved in the opposite direction, declining over the study period.

The age pattern is perhaps the most actionable result. Across all regions, ADHD-related disability rose through childhood, peaked at ages 10 to 14, and then declined progressively, with the peak particularly pronounced in East Asia. Males consistently showed higher rates than females across regions and age groups. The authors cautioned that this male predominance should not obscure girls, whose symptoms may be less readily noticed, particularly inattentive presentations that do not disrupt classrooms in obvious ways.

The implications extend to how health systems and schools identify and support affected young people. The concentration of disability in early adolescence makes timely recognition especially important, and the researchers argue that schools can train teachers to recognize diverse ADHD presentations, strengthen referral pathways, and invest in classroom management and social-emotional learning around this age window. Clinicians can prioritize early assessment in this group, attend to sex-specific symptom profiles, integrate screening into primary care, and connect families with evidence-based behavioral support.

The study also examined changes among children aged 0 to 14 before the pandemic, from 2015 to 2019, and during the subsequent period, from 2019 to 2023. Most countries showed increases, with the Republic of Korea shifting from a decline to a marked rise. The authors stressed that these patterns cannot establish that the pandemic caused the increase, and should be read as observed trends rather than proof of a single cause.

Socioeconomic context adds another layer. Higher scores on the Socio-demographic Index were associated with higher estimated ADHD disability, a relationship that may partly reflect differences in recognition and access to diagnosis rather than underlying disease alone. The team emphasized that better detection, service access and reporting can all influence estimated burden, so the figures should guide planning and monitoring rather than rank health systems. Asia-wide averages, they warn, can obscure important national differences that need to be interpreted alongside local diagnostic and service contexts.

For policymakers, the regional contrasts offer a map for targeting resources, emphasizing early identification and timely treatment, and expanding school- and primary-care-based services where specialist capacity is limited. The central message, the authors said, is not simply that some countries have higher estimates than others, but that the burden follows recognizable patterns that can inform support.
