Avident Health LLC and Unite for HER have been awarded an independent quality improvement grant funded collaboratively by Pfizer Inc. and the National Comprehensive Cancer Network (NCCN). The grant will support a collaborative project deploying Avident Health's proprietary MyNavigator digital platform to enhance care delivery for patients diagnosed with metastatic breast cancer (mBC). This initiative matters because it addresses critical gaps in ongoing treatment management, where adherence, symptom monitoring, and whole-person supportive care are paramount but often difficult to maintain outside clinical appointments.
Living with metastatic breast cancer requires complex, continuous care, yet patients and their loved ones frequently struggle to navigate clinical guidelines and access community-based supportive resources. The MyNavigator platform aims to bridge these communication gaps by serving as an interactive, home-based companion. It delivers tailored education aligned with NCCN Guidelines for Patients, translating complex clinical pathways into accessible, bite-sized modules tailored to each patient's specific treatment journey. This empowers patients to participate actively in informed clinical decision-making with their care teams.
Beyond education, the platform streamlines access to integrative supportive care by directly integrating Unite for HER's comprehensive suite of free services—including professional nutrition counseling, fresh food delivery, mental health counseling, and evidence-based integrative therapies—into daily navigation prompts. This integration is designed to drive active enrollment and utilization of these services, removing barriers that often prevent patients from accessing vital support. Unite for HER, a national non-profit, provides these services at no personal cost to breast and ovarian cancer patients, and the collaboration enables direct reach to patients and families at the moment they need guidance.
A distinctive feature of the initiative is its engagement of informal family caregivers. The platform uniquely includes family members and daily care partners in a secure, proactive communication loop, ensuring they have the context, education, and early symptom alerting needed to support treatment adherence at home. This caregiver-inclusive approach fosters shared decision-making and enhances quality of life for both patients and their support networks. Additionally, the platform captures patient-reported check-ins and symptom trends to alert clinical teams early, enabling proactive interventions that can prevent avoidable complications and reduce acute care utilization.
The project will measure key quality improvement endpoints over the grant period, including improvements in patient knowledge concordance with NCCN Guidelines, rates of supportive service utilization, patient-reported decisional conflict, and therapy adherence metrics. These measurements will provide evidence of the platform's impact on care delivery and patient outcomes. By combining digital coordination, integrative services, and gold-standard guidelines, the collaboration aims to create a continuous circle of support that empowers patients, supports clinical teams, and engages family caregivers.
