The global burden of gout is set to expand dramatically over the next quarter-century, with projections indicating cases will rise significantly. In the United States alone, over 9 million people have been diagnosed with the disease, and an even larger pool of 44 million people have uric acid levels above the normal range, placing them at risk. This growth trajectory underscores an urgent need for better treatment options, as existing therapies often come with severe gastrointestinal issues, kidney toxicity, and cardiovascular risks that hinder patient adherence.
XORTX Therapeutics Inc. (NASDAQ: XRTX) is positioning itself to meet that need with XORLO, a proprietary formulation of oxypurinol. According to the company, XORLO uses a liposomal delivery system to overcome the historically poor oral absorption of oxypurinol, enabling effective uric acid reduction at substantially lower total daily doses. This approach aims to minimize the toxic-level exposure and severe gastrointestinal side effects associated with high-dose traditional therapies. Current standard-of-care oral xanthine oxidase inhibitors carry boxed warnings for cardiovascular risk and are intolerable for approximately 3% to 5% of patients.
The company recently announcing the launch of drug material and tablet manufacturing to support a planned two-part clinical study, XRX-OXY-102. Contract production of pure oxypurinol drug substance using XORTX's proprietary synthetic pathway has begun, and the company is working with its contract manufacturing partner toward commercial drug batches. The study is being advanced based on positive prior clinical results and discussions with the FDA.
XORTX is also working toward a new drug application submission for XORLO. The company has prepared a new Investigational New Drug application for submission to the FDA in the fourth quarter of 2026, describing its clinical development plan. If the two-part trial is completed successfully, XORTX intends to request a pre-NDA meeting and prepare an NDA submission while engaging in pre-commercialization activities. The company is targeting an NDA submission in about one year and estimates a market opportunity of $700 million annually for XORLO.
"Our planned IND submission to the FDA next quarter is a defining step for the XRx-026 program," said Allen Davidoff, Co-CEO of XORTX. "It reflects the strength of our prior clinical results and moves us closer to potentially offering gout patients a differentiated treatment option."
The implications of these milestones extend beyond XORTX. With gout prevalence expected to climb sharply, a better-tolerated therapy could improve adherence and outcomes for millions. For investors, the progress toward an NDA represents a critical value inflection point. As XORTX continues to advance XORLO, the biotech sector will be watching whether it can deliver on its promise to reshape the gout treatment landscape.
